For families & young adults
Your first visit on your own
A guide for ages 18–26. You do not need to have every answer before asking for care. Start with what matters most to you.
Before the visit
- Write your three most important questions.
- List medicines and supplements, including doses if you know them.
- Gather existing test results or records if available; ask how to send them securely.
- Share your insurance card with our team so we can check benefits, referral requirements and expected costs.
- Choose a private place for a video visit and test your connection.
Your care. Your choices.
Choose how we contact you and who, if anyone, joins your care. Ask about permission to share information and what insurance notices may reveal if you use someone else’s plan.
Leave with a clear next step
- What is the plan, and what should I track?
- Who handles refills, and how early should I request them?
- Which symptoms mean I should call sooner?
- When is my next visit, and what should happen before it?
- How will we plan my transition to ongoing adult care?
