Your visit

Care that supports your independence

Plan around school, work and daily life. Understand your options and choose manageable steps. Use your own contact details and tell us how to reach you.

You choose who joins your visits and who we can talk to about your care. A parent or partner can be part of it if you want them there.

Bring these to the conversation

  • Your routine: school, work, meals and any medicines
  • Your goals and the questions on your mind
  • Insurance questions, especially if your plan is changing
  • How you’d like to manage your care as an adult

Growing up with us

Keep building your independence

We care for young adults ages 18–26. As your needs change, we help plan the next step in your care.

  1. You lead your visits

Request your own visits and talk with your clinician directly. Family can join if you choose.

  2. Build your skills

Learn about your diagnosis, your medicines and refills, booking visits and how your insurance works.

  3. Plan your next steps

Talk with your clinician about records and a transition plan when you need another care team.

Questions from young adults

Contact our team
Do I need a parent to request a visit?

No. If you’re 18–26, you can request care for yourself using your own contact information. Call us at (571) 480-6053 if you need help with the request.

Can my parent still come to my visits?

Yes, if you want them to. You choose who joins your visits and who we can talk to about your care.

What happens when I need another care team?

Discuss the transition with your clinician, including which records and treatment information the next clinician will need. Plan ahead as you approach age 27 or if your care needs change.

Can I ask about weight medication?

Yes. Your clinician reviews whether medication fits your health, goals and medical history. If it is prescribed, we help you understand coverage. Explore medication options and safety.

Can I use my parent’s insurance?

Yes, if you are covered by that plan. Share your insurance card with us and we’ll check your visit benefits and expected costs. Coverage depends on the plan and network.
